All-on-Four and full-arch implants in Beylikdüzü: the road to fixed teeth

All-on-Four means placing four implants in a toothless jaw and fixing a one-piece row of teeth on top. If bone and general health allow, temporary fixed teeth can be fitted within a few days of implant surgery, often the same day; the final teeth are made after the implants have fused with the bone, usually within 3–6 months. At Dent32, the decision is made after a CT scan to assess the bone.

What is All-on-Four, and how is it different from dentures?

A conventional full denture rests on the palate or gums and can move in the mouth. With All-on-Four, the prosthesis is screwed onto four implants; you don't take it in and out but clean it as you would your teeth. The back implants are usually placed at an angle, which keeps them away from the sinus and nerve and can reduce the need for bone grafting.

Who is it suitable for?

  • People who have lost all their teeth, or whose remaining teeth can't be saved
  • Denture wearers who want fixed teeth
  • People whose general health allows minor surgery
  • People with enough bone to support at least four implants

Uncontrolled diabetes, heavy smoking and some bone medicines increase the risk; these are assessed in detail beforehand. See who can get dental implants. If bone is very limited, more than four implants (All-on-Six) or bone augmentation may be needed: bone grafts and sinus lifts.

Step by step

  1. Examination and 3D CT scan: Bone volume and distance to the sinus and nerve are measured.
  2. Digital planning: Implant positions and the length, colour and smile line of the teeth are planned.
  3. Surgery: Any extractions and placement of the four implants are done under local anaesthetic, usually in one session.
  4. Temporary fixed teeth: If the implants are stable enough at placement, a temporary row of teeth is fitted within a few days, often the same day.
  5. Integration: A soft diet for about 3–6 months.
  6. Final prosthesis: Impressions, try-in and fitting of the final row of teeth.

Care and check-ups

Food can collect under a fixed prosthesis. You need to brush twice a day, clean under the prosthesis with an interdental brush or water flosser, and have professional check-ups at least once or twice a year. At check-ups we monitor screw tightness, the gums and the bone around the implants. Inflammation around implants (peri-implantitis) is the main long-term threat; see how long do dental implants last for details.

All-on-Four planning at Dent32

At the examination we look at your bone, your expectations and your budget together, and set out All-on-Four, implant-retained removable dentures and conventional dentures side by side. The main cost factors are the implant brand, the prosthesis material (acrylic, zirconia or metal-reinforced) and any additional surgery. See implant-supported dentures and implant cost factors.

Frequently asked questions

How many days does All-on-Four take?

Temporary fixed teeth are usually fitted within a few days of surgery. The final prosthesis is made after the implants fuse with the bone, usually within 3–6 months.

Is All-on-Four painful?

Surgery is done under local anaesthetic. There is swelling and tenderness for a few days afterwards, controlled with painkillers.

Can I have All-on-Four if I don't have much bone?

Angling the back implants often reduces the need for bone grafting. If bone is very insufficient, extra procedures or a different plan may be needed.

Can both jaws be done at the same time?

Yes. If your general health allows, both jaws can be planned in the same session or a short time apart.

How long does a fixed prosthesis last?

With good care, the implants can last for many years. The prosthetic teeth may wear over time and need replacing.
