Tooth extraction and impacted wisdom teeth in Beylikdüzü: procedure, pain and recovery

A tooth extraction is the removal, under local anaesthetic, of a tooth that is too damaged to save or is causing problems. A routine extraction usually takes a few minutes; an impacted wisdom tooth usually takes 20–45 minutes. You won't feel pain during the procedure; mild pain and swelling for the first 2–3 days is normal. At Dent32, we only decide on extraction after discussing options for saving the tooth.

When does a tooth need to be extracted?

We aim to keep teeth wherever possible; decay or infection can often be treated with root canal treatment and a crown. Extraction usually comes up when:

  • The root is fractured or decay has reached the root and the tooth can't be restored
  • Advanced gum disease has destroyed the bone support and the tooth is loose
  • An impacted wisdom tooth causes repeated infection, decay in the neighbouring tooth or a cyst
  • Space is needed for orthodontic treatment
  • Infection persists despite treatment

How is a tooth extracted, and does it hurt?

First, an X-ray is used to assess the root shape and surrounding structures. Local anaesthetic numbs the area completely; you may feel pressure but no pain. In a routine extraction, the tooth is gently loosened from its socket with special instruments and removed.

For impacted teeth or teeth with curved roots, the gum is opened, the tooth may be divided into pieces to remove it, and the area is closed with stitches. When patients tell us "I didn't feel a thing", it's mostly down to giving enough anaesthetic at the right time.

Blood thinners, diabetes and pregnancy

  • Blood thinners: Don't stop aspirin, warfarin or newer anticoagulants on your own. The decision is made with the doctor who prescribed them; most extractions can be done safely while continuing the medicine, with local bleeding control.
  • Diabetes: If your blood sugar is under control, extraction is done as normal. Poorly controlled diabetes can delay healing.
  • Osteoporosis medicines: If you have taken bisphosphonates or similar medicines, always tell us; the plan is adjusted accordingly.
  • Pregnancy: Non-urgent extractions are usually postponed to the second trimester. Details: dental treatment during pregnancy.

After the extraction

  • Bite on the gauze pad for 30–45 minutes.
  • For the first 24 hours, don't spit, use a straw or rinse; the clot could be dislodged.
  • Don't eat until the numbness wears off, and choose lukewarm, soft foods on the first day.
  • Smoking delays healing and raises the risk of dry socket; avoid it for at least 2–3 days.
  • Severe pain that gets worse after day 2–3, with a bad smell, may be dry socket; contact us.

What replaces an extracted tooth?

A gap can be filled with an implant, a bridge or a removable denture. In suitable cases an implant can be placed in the same session as the extraction. We compare the options in implant or bridge?.

Does a tooth extraction hurt?

After a routine extraction, most people go back to work the same day or the next day. After an impacted wisdom tooth, resting for 1–2 days is a good idea.

When are the stitches removed?

If dissolving stitches weren't used, usually after 7–10 days.
