Problems with implant dentures: looseness, sore spots, worn clips, breakage and odour

The most common problems with implant dentures are looseness as the clips wear, sore spots on the gums, loose screws in fixed bridges, chipped or broken teeth, and odour and inflammation where cleaning is difficult. Most are solved with simple adjustments or part replacement, and coming in early protects both the denture and the implants. This article covers the signs and what to do.

Looseness in clip-on (removable) dentures

In implant-retained removable dentures, the plastic parts that provide grip wear a little each time the denture is taken out and put in. Depending on use, they usually need replacing every 6–18 months. It's a quick procedure and doesn't require a new denture. Taking the denture out at night and cleaning it also protects both the gums and the parts. More: what is an implant-supported denture.

Loose screws and breakage in fixed bridges

In screw-retained fixed bridges, a slight clicking or movement when chewing usually means a screw has loosened. Don't wait when you notice it: a loose screw can eventually break or strain the implant components. Teeth break more often on acrylic bridges than on zirconia; for people who grind, a night guard lowers the risk. On materials: zirconia on a titanium bar.

Odour, bleeding and inflammation around implants

If the underside of a fixed bridge and the area around the implants aren't cleaned well, food debris and plaque build up. The first signs are odour, bleeding when brushing and redness of the gums. Left untreated, bone loss around the implants (peri-implantitis) can develop. Interdental brushes, a water flosser and superfloss, plus a professional clean every six months, greatly reduce this risk. See how long implants last and peri-implantitis.

When to come in without waiting

  • Swelling, pus or fever at an implant
  • The denture suddenly becoming noticeably loose
  • Part of a fixed bridge coming off
  • A feeling that one implant is moving on its own

We can also examine and maintain dentures made at another clinic; if possible, bring documents showing the implant brand and sizes.

Frequently asked questions

Why is my implant denture loose?

In removable dentures the most common cause is worn clip inserts; in fixed bridges, a loose screw. Both are usually fixed with a short procedure.

How often do the clips need replacing?

Usually every 6–18 months, depending on use. Replacing them doesn't require a new denture.

Can you maintain an implant denture made elsewhere?

Yes. Examination, cleaning and many adjustments are possible. Replacing parts requires knowing the implant brand and size, so bring any documents you have.

There's a smell from under my fixed bridge. What should I do?

There may be build-up under the bridge or gum inflammation. You need a professional clean and examination; using interdental brushes and a water flosser at home prevents it coming back.
