Is pain after root canal treatment normal? How long it lasts and when to go back
For a few days after root canal treatment, mild to moderate pain and tenderness, especially when biting, is normal and is usually controlled with painkillers. For most patients it eases noticeably within 2–3 days and is gone within a week. Pain that keeps getting worse, facial swelling or fever is not expected, and you should go back to your dentist.

Why is there pain after a root canal?
Root canal treatment removes the inflamed nerve tissue inside the tooth, but the tissues around the root tip can be affected by the procedure. If there was inflammation at the root tip beforehand or the tooth was very painful, tenderness afterwards may be a little more noticeable. This doesn't mean the treatment has failed; it's the tissues' healing response.
A normal recovery timeline
| Time | What to expect | What to do |
|---|---|---|
| First 24 hours | An ache once the numbness wears off, tenderness on biting | Painkiller, don't chew on that side |
| Days 2–3 | Tenderness eases noticeably | Soft foods |
| 1 week | Most patients have no symptoms | Appointment for the final filling or crown |
| 6–12 months | If there was an inflamed area at the root tip, the bone heals | Follow-up X-ray |
Does the second visit hurt?
Root canal treatment can be done in one or two visits. With two visits, the canals are cleaned and a temporary medicine is placed at the first; at the second they are filled permanently. The second visit is usually more comfortable than the first, and local anaesthetic is given again if needed. For the number of visits and the process, see root canal treatment in Beylikdüzü.
What to watch after treatment
- Don't eat until the numbness wears off.
- Don't chew hard food on that side until the final filling or crown is done; a root-treated tooth is fragile.
- The temporary filling may wear a little; if it falls out or a large piece breaks off, call the clinic.
- Don't delay the final restoration. Even a well-treated tooth can be lost to leakage or fracture if it isn't sealed.
When to go back to your dentist
- Pain is getting worse instead of better
- Swelling has appeared in the gum or face
- You have a fever
- Severe throbbing even without touching the tooth
- The tooth feels 'high' when you bite (the bite may need adjusting)
If a root-treated tooth starts hurting again months or years later, see root canal retreatment.

Frequently asked questions
How many days does pain last after a root canal?
Mild pain and tenderness on biting usually ease noticeably within 2–3 days and are gone within a week. Increasing pain or swelling is not normal.
How long does a root canal take to heal?
Pain and tenderness settle within a few days. If there was an inflamed area at the root tip, the bone can take 6–12 months to heal, which is checked with a follow-up X-ray.
Do I need antibiotics after a root canal?
Usually not. Your dentist will prescribe them if needed for spreading infection, swelling or fever.
Do I need a crown after a root canal?
For back teeth and teeth that have lost a lot of structure, a crown is often advised to prevent fracture. For front teeth with little loss, a filling may be enough.
