What causes a white tongue? Coating, oral thrush and how to clean your tongue
A white tongue is usually caused by bacteria, dead cells and food debris building up between the small projections (papillae) on its surface. Dry mouth, smoking, mouth breathing and feverish illness make this build-up worse. Regular tongue cleaning and plenty of water usually clear it. A white coating that leaves red, raw areas when wiped suggests thrush, and white patches that won't wipe off at all need to be examined.

Causes of a white tongue
| Condition | Appearance | What to do |
|---|---|---|
| Coated tongue (build-up) | White-yellowish coating on the back and middle of the tongue that wipes off | Tongue cleaning, water, treating dry mouth |
| Oral thrush (fungal) | Red underneath when wiped, sometimes burning; after antibiotics, steroid inhalers, dentures, diabetes | Examination and antifungal treatment |
| Leukoplakia | A white patch with clear edges that won't wipe off | Always examine; biopsy if needed |
| Geographic tongue | Shifting red areas with white borders | Usually harmless; monitored |
How to clean your tongue
- Place a tongue scraper or soft brush at the back of the tongue.
- Draw it forward with light pressure; don't press hard.
- Rinse the cleaner after each stroke; 3–5 strokes are enough.
- Do it once a day, ideally with your morning brush.
- If you gag easily, hold your breath, stick your tongue out and make short strokes.
Tongue cleaning and bad breath
Much of the odour-causing compounds in bad breath are produced by bacteria on the back of the tongue, so regular tongue cleaning can reduce bad breath noticeably. If it persists, other causes such as gum disease, decay or build-up under crowns should be looked for. See causes and treatment of bad breath.
When to get it checked
- A white coating that hasn't cleared in two weeks despite cleaning
- Patches that won't wipe off or bleed when wiped
- Burning, pain or a sore on the tongue
- New white or red patches in people who smoke or drink alcohol

Frequently asked questions
Why is my tongue white?
The most common cause is bacteria, dead cells and food debris building up on the tongue's surface. Dry mouth, smoking and feverish illness make it worse. Tongue cleaning and plenty of water usually clear it.
Tongue scraper or toothbrush?
A tongue scraper cleans a wider area with less gagging; a soft toothbrush also works if you don't have one. What matters is gentle, regular cleaning.
How can I tell if my white tongue is thrush?
With thrush, the white coating leaves a red, tender surface when wiped and often follows antibiotics, steroid inhalers or denture use. An examination is needed to be sure.
